Chapman Partnership welcomes individuals and families through a structured assessment and referral process led by our Community Case Management (CCM) team. This team works to understand each person's needs and connect them with the most appropriate services, whether that's housing, healthcare, or support for families.
Note: While we offer many services, it’s important to know that hotel vouchers and immediate shelter are not direct services we provide. When we are not the right fit, we do our best to refer people to other resources in the community that may be able to help.
