Assessment Process & Intake Process

Chapman Partnership welcomes individuals and families through a structured assessment and referral process led by our Community Case Management (CCM) team. This team works to understand each person's needs and connect them with the most appropriate services, whether that's housing, healthcare, or support for families.


Note: While we offer many services, it’s important to know that hotel vouchers and immediate shelter are not direct services we provide. When we are not the right fit, we do our best to refer people to other resources in the community that may be able to help.

Ongoing Support & Follow-Up Services

Our commitment doesn’t end when clients secure housing. Chapman Partnership provides follow-up services to help ensure long-term stability, offering continued guidance and essential resources. Community members can support this effort by donating move-out kits filled with household essentials to help individuals transition into independent living.

As the private sector partner of the Miami-Dade County Homeless Trust, Chapman Partnership is another step in the continuum of care. Any person or family who is homeless, about to be homeless, or assisting someone facing homelessness, and requiring emergency assistance must first contact the Homeless Helpline administered by the Miami-Dade County Homeless Trust.
